By submitting this registration form, you agree to attend all required live sessions and to complete all course assignments. Course assignments require the student to read all instructions and follow them as detailed. Also, by submitting this registration form, you agree that you will be held to all attendance requirements; missing 2+ hours will require the student to attend next cohort or forfeit the attended time associated with this registration and/or missing up to 15 minutes will require the student to complete and submit additional course work to make their time up. All birth workers and healthcare providers are advised to obtain reliable backup as special attendance accommodations cannot be made for births.
